Sign Up
There are two easy ways to sign up
- Through The Office
Ask our receptionist for a registration form, complete it and leave it with the office staff.
You will receive an email from:
[email protected]Click on the link provided in the email and follow the instructions.
- Through the Website
Visit scmg.followmyhealth.com
(Note: do not type “www” in front of scmg.followmyhealth.com, it will direct you to the wrong site)Click the Create an Account button.
Fill in some basic information and create a secure User Name and Password
Search for your doctor and request to connect.
You will receive an email confirmation once your account has been verified and created.
It may take several days to process new registration requests. It is recommended that you check your “Junk” or “Spam” folder for the invitation from [email protected]. Occasionally, emails go into these folders unintentionally.
